Barbershop Conversations Consent
I acknowledge and understand that my child’s participation in Barbershop Conversations is of no charge to me or my child and is completely voluntary.
I acknowledge and understand that my child must be between the ages of 12-18.
I acknowledge and consent that my child’s video, statements, and or pictures may or may not be used for, but not limited to: advertising, commercial, promotional and or marketing purposes by WeFAM, Inc.
I acknowledge and understand that Barbershop Conversations are recorded live and it is imperative to arrive 15 minutes before scheduled time.
I acknowledge and understand that a WeFAM Voucher will be issued to my child (in attendance) for a free haircut that must be redeemed within 30 days of issuance at the participating vendors listed on the back.
I acknowledge and understand that as a parent/legal guardian, I am welcome to stay during the Barbershop Conversations while practicing social distancing and wearing a face mask.
