Concerning the Distribution of WeFAM Vouchers

  • This Memorandum Of Understanding (MOU), (hereinafter referred to as Memorandum) is made and entered into on:
  • Date Format: MM slash DD slash YYYY

  • by and between WeFAM, Inc. residing at 478 Greenpointe Parkway, Fort Wayne, Indiana 46814, (hereinafter referred to as the “First Party”) and:

  • (hereinafter referred to as the “Second Party” and collectively known as the “Parties”) for the purpose of establishing and achieving various goals and objectives relating to the partnership.

    WHEREAS, the aforementioned Parties desire to enter into the herein described agreement in which they shall work together to accomplish the goals and objectives set forth;

    AND WHEREAS, the Parties are desirous to enter an understanding, thus setting out all necessary working arrangements that both Parties agree shall be necessary to complete this partnership;


    MISSION

    The aforementioned partnership has been established with the following intended mission in mind:


  • How Many men, women, boys, girls do you serve on a yearly basis

  • GOALS AND OBJECTIVES

    The Parties to this Memorandum shall endeavor to work together to achieve the following goals and objectives:

    To establish a system for distributing WeFAM vouchers to consumers registered with Our Company.


    RESPONSIBILITIES OF WeFAM, Inc.

    WeFAM, Inc. shall perform the following obligations:

    1. WeFAM, Inc. will be responsible for the collection of monetary grants and donations, as well as fundraising.

    2. WeFAM, Inc. will be responsible for distributing WeFAM vouchers via email to the requesting staff member upon obtaining necessary funds to fulfill request

    3. WeFAM, Inc. will recruit all licensed grooming professionals to perform all services at State Board regulated beauty and barber colleges, barbershops, and beauty salons. These services include but not to;

    a. Washing hair

    b. Drying hair

    c. Styling hair

    d. Cutting hair

    e. Manicure/pedicure

    f. Facials/massage

    4. WeFAM, Inc. will produce and manufacture all WeFAM Vouchers.

    5. WeFAM Inc. will be responsible for covering the cost of services and rendering payment to registered and approved vendors.



    RESPONSIBILITIES OF OUR COMPANY

    Our Company shall perform the following obligations:

    1. Our Company will be responsible for submitting a logo to be used on WeFAM marketing collateral.

    2. Upon delivery of the WeFAM Vouchers, Our Company shall appoint one employee to monitor the WeFAM Vouchers and the WeFAM Voucher Request Sheets.

    3. A parent or legal guardian (“requestee”), who is over the age of eighteen, must request a WeFAM Voucher through the WeFAM Voucher Request Sheet.

    4. The WeFAM Voucher Request Sheet shall include the name of the individual being serviced, the birthdate of the individual being serviced, the address of the individual being serviced, the contact information of the parent or guardian, the service being requested, and four certifications to be initialed by the requestee.

    5. Upon completion of the WeFAM Voucher Request Sheet, the appointed employee shall confirm the identifier number on the WeFAM Voucher Request Sheet and the WeFAM Voucher are identical.

    6. The appointed employee shall then issue the WeFAM Voucher to the requestee. (note - expiration date)

    7. My Company shall submit a copy of the WeFAM Voucher Request sheet via email to: wefaminc@gmail.com, subject: Return Voucher Request Sheet once WeFAM Vouchers have been distributed.


    TIMELINE

    The above outlined goals and objectives shall be contingent on the Parties obtaining the necessary funds required for operation. Upon obtaining necessary funds, WeFAM Vouchers will be produced with a 30 day expiration date from the time of production. It is highly encouraged to distribute WeFAM Vouchers as quickly as possible.


    TERMS OF UNDERSTANDING

    The terms of this Memorandum of Understanding shall last the lifetime of the partnership from the aforementioned effective date.


    CANCELLATION OF THIS MEMORANDUM

    This Memorandum of Understanding may be amended or modified at any time in writing by mutual consent of both parties.

    In addition, the Memorandum of Understanding may be cancelled by either party with 30 days advance written notice, with the exception where cause for cancellation may include, but not limited to, a material and significant breach of any of the provisions contained herein, when it may be canceled upon delivery of written notice to the other party.


    GENERAL PROVISIONS

    The Parties acknowledge and understand that they must be able to fulfill their responsibilities under this Memorandum of Understanding in accordance with the provisions of the law and regulations that govern their activities. Nothing in the Memorandum is intended to negate or otherwise render ineffective any such provisions or operating procedures. The parties assume full responsibility for their performance under the terms of the Memorandum.

    If at any time either party is unable to perform their duties of responsibilities under this Memorandum of Understanding consistent with such party’s statutory and regulatory mandates the affected party shall immediately provide written notice to the other party to establish a date for resolution for the matter.


    LIMITATION OF LIABILITY

    No rights or limitation of rights shall arise or be assumed between the Parties as a result of the terms of the Memorandum of Understanding.


    SIGNATORIES

    This Memorandum of Understanding shall be signed on the behalf of WeFAM, Inc. by Dr. Juan Williams, Founder/CEO and on the behalf of Our Company
